Under section 503A, a pharmacy may compound from a bulk substance only if it has a USP or NF monograph, is a component of an FDA-approved drug, or appears on the FDA's 503A bulks list. Ask which category each substance falls under before building a protocol around it. Compounds outside all three cannot be prepared, whatever their popularity.
Shortage status decides what can be compounded
When a drug comes off the FDA drug shortage list, a 503A pharmacy may no longer compound what is essentially a copy of the commercially available product. A compounded prescription then needs the prescriber's determination that a specific change makes a significant difference for the identified patient, and that determination belongs in the chart for every prescription.
