Buying exosomes wholesale for Honolulu practices
Once a Honolulu practice passes wholesale account verification, it may purchase biomolecular signaling vesicles (exosomes) for topical aesthetic use by licensed practitioners. The cartons are frozen and packed in Florida, flown from the mainland to Oahu, and driven by the carrier to addresses across the City and County of Honolulu. Compounded 503A preparations are not offered yet to Honolulu or any other Hawaii address, because our pharmacy partner's Hawaii licence is still pending.

When an order reaches a Honolulu practice
A Honolulu carton crosses the Pacific by air, lands on Oahu and finishes on island roads; the panel above gives the transit window. The run from the airport into town follows Nimitz Highway or the H-1 Freeway. For the week of 10 to 16 October 2026, HDOT listed nightly closures of two to three eastbound H-1 lanes between the Kaonohi Street overpass and the Valkenburgh Street underpass, from 8:30 p.m. to 4 a.m., for the Salt Lake Rehabilitation project, and morning right lane closures on Nimitz Highway between Valkenburgh Street and Lagoon Drive.
Inside the Nimitz Highway and Ala Moana corridor, HDOT keeps Awa Street closed from Nimitz and the inside lane of Nimitz shut in both directions between Awa and Richards Streets until further notice, which slows deliveries toward Kakaako and downtown.
Storms close whole routes. During Tropical Storm Lala on 16 August 2026 the City counted more than 20 road closures, Pali Highway, Dillingham Boulevard and Kalakaua Avenue among them. A Honolulu parcel caught behind those closures stays with the carrier until the road is passable again.
Cold-chain receiving at a Honolulu address
Honolulu heat is steady rather than extreme, yet a frozen box on an open lanai or a parking structure loading bay in August still loses dry ice fast. During Tropical Storm Lala the City reported tens of thousands of Hawaiian Electric customers without power, so a practice holding stock keeps its freezer on backup power and signs up for HNL Alert well before the Central Pacific hurricane season, which the City notes runs through 30 November.
At a medical office tower near Punchbowl Street or along Kapiolani Boulevard, tell us whether the driver uses a loading dock or the lobby desk, and who signs. That person opens the shipper where air moves freely, letting carbon dioxide disperse, checks for leftover dry ice, notes when it came and its lot number, then freezes the vials: the manufacturer's handling guidance gives -20°C for stock kept long and 4°C for vials already thawed. A carton reaching a Honolulu suite with its dry ice gone is reshipped fresh.
Rules that apply to a Honolulu order
- A practice doing business in Honolulu registers with the Hawaii Department of Taxation for a General Excise Tax licence, a one time $20 fee, filed online through Hawaii Tax Online or on Form BB-1. Business on Oahu is taxed at the 4% state rate plus the City and County of Honolulu surcharge of 0.5%, in force from 1 January 2007 to 31 December 2030.
- Building and occupancy questions go to the City Department of Planning and Permitting at 650 South King Street. A building permit is needed to alter, convert, move or repair a building, and DPP requires an approved Sewer Capacity Analysis before a change of occupancy or use, so a Honolulu suite converted from retail or office use joins the account once those approvals are complete.
- For storms, the City Department of Emergency Management speaks for Oahu streets and shelters, the Hawaii Emergency Management Agency coordinates statewide, and the National Weather Service office in Honolulu issues the watches and warnings.
Communities near Honolulu and county rules
Hawaii has no cities or towns below the county, so Kailua, Kaneohe, Kapolei and Pearl City all sit inside the City and County of Honolulu and follow the same Department of Planning and Permitting and the same 0.5% Oahu surcharge. The nearest separate rules are on Maui, where the County of Maui Development Services Administration in Kahului issues building permits for the construction, alteration and use of any building in that county, and where the county surcharge of 0.5% began only on 1 January 2024. A Honolulu permit or occupancy approval does not cover a Maui suite, though the statewide General Excise Tax licence does.
Honolulu questions
- Our Kailua clinic is on the windward side of the Pali Highway. What happens when storms close the Pali on a Honolulu delivery day?
- Kailua is inside the City and County of Honolulu, so the same lane and transit window apply, but the Pali Highway was among the closures during Tropical Storm Lala in August 2026. If the Pali shuts again, the carrier holds the carton until the highway is passable, while we follow its tracking and keep your Kailua recipient informed by phone.
- Our Kakaako office is just off Ala Moana Boulevard near Nimitz Highway. Do the HDOT corridor closures matter?
- HDOT keeps the inside lane of Nimitz Highway closed in both directions between Awa and Richards Streets, with Awa Street shut from Nimitz, until further notice. Give the driver a clear entrance on a side street and a recipient who answers the phone, because lane closures in this corridor leave drivers little room to wait at the curb.
- We are adding a second suite on Maui after opening in Honolulu. Do we need a new Hawaii tax licence?
- The Hawaii General Excise Tax licence is issued statewide, so one licence covers both islands, but building permits for the Maui suite come from the County of Maui Development Services Administration in Kahului, not from Honolulu DPP. Send us the Maui address and receiving contact, and each lot is logged against whichever suite signed for it.
