Levothyroxine and liothyronine are iodinated tyrosine derivatives separated by a single iodine atom. T4 circulates largely as a prohormone, and which iodine a deiodinase removes decides whether it becomes active T3 or inactive reverse T3.

Biosynthesis, Handling & Sourcing

Thyroid hormones are among the very few iodinated molecules in human biochemistry. Both are built from two linked tyrosine rings and differ by one atom: thyroxine carries four iodines, liothyronine three. Which iodine comes off decides everything that follows. Removing one from the outer ring converts T4 into T3, the form with far greater affinity for the nuclear thyroid hormone receptor. Removing one from the inner ring instead yields reverse T3, which is inert. The same molecule therefore becomes either the active hormone or a dead end depending on which deiodinase reaches it, and those enzymes are distributed unevenly across tissue.

That branch point is why a combination preparation exists at all. Most of the T3 circulating in a healthy adult arises from peripheral deiodination rather than from direct thyroid secretion, so a T4-only regimen depends entirely on that conversion behaving as expected. Consensus documents from the American Thyroid Association describe the evidence for combination therapy as unresolved rather than settled, with patient selection named as the open question. What a compounded preparation adds is that a prescriber can specify a ratio instead of choosing among the ratios a manufacturer happens to make.

The strength ladder on this page looks arbitrary and is not. Eight of the ten listed strengths hold a T4 to T3 ratio of about 4.2 to 1, and their T4 content steps as 19, 28.5, 38, 57, 76, 95, 114 and 152 micrograms. Divide any of those by 38 and a clean figure falls out: one half, three quarters, one, one and a half, two, two and a half, three, four. Thirty-eight micrograms of T4 alongside nine of T3 is the content conventionally quoted for one grain of desiccated thyroid, so this ladder is a grain scale written in micrograms. The two remaining strengths break the pattern on purpose, sitting at round T4 values with ratios nearer 5 to 1 and 3.3 to 1. A practice coming from a desiccated product will recognise the first eight on sight; one starting fresh will find the last two easier to reason about.

Levothyroxine is the textbook narrow therapeutic index drug, and that is a manufacturing statement as much as a clinical one. The active occupies a very small fraction of what fills a capsule, so whether every capsule in a fill contains what the label says depends on how evenly the drug is distributed through the powder blend rather than on anything visible afterwards. Levothyroxine sodium is also hygroscopic and light sensitive, and its stability in solid form has drawn sustained regulatory attention precisely because a small potency shift matters more here than it would for most molecules.

These are non-sterile immediate-release capsules compounded under USP General Chapter 795. The formulary lists quantity per fill rather than a fixed beyond-use date for this line, so the date assigned at preparation is the one that governs and it is printed on the container. T3 clears considerably faster than T4, which is why immediate-release and sustained-release versions of these combinations both exist and are not interchangeable; this line is immediate release.
