Biosynthesis, Handling & Sourcing

DHEA is made in the zona reticularis of the adrenal cortex, where CYP17A1 acts twice on pregnenolone, first as a hydroxylase and then as a lyase, to cut the C17 side chain and leave a C19 steroid. Almost all of what circulates is not DHEA itself but its sulfate ester, DHEA-S, which persists in plasma far longer than the free form. The two interconvert continuously through sulfotransferase and sulfatase activity, so the sulfate behaves as a circulating reservoir and a single free-DHEA measurement describes a moment rather than a supply.

The steroid is best understood as a precursor rather than as an active hormone. It has little affinity for the androgen or estrogen receptor on its own, and what activity is described in the endocrine literature follows conversion inside peripheral tissue by 3-beta-hydroxysteroid dehydrogenase, 17-beta-hydroxysteroid dehydrogenase and aromatase. Because those enzymes are expressed unevenly, the same circulating concentration produces different local steroid exposure in different tissues. Labrie named this arrangement intracrinology, and it is the reason serum values and tissue-level activity are described in the literature as loosely coupled.

Compounding considerations are those of any potent bulk steroid in a capsule. The pharmacy works from bulk active rather than from a manufactured tablet, so content uniformity through the powder blend is what determines whether every capsule in a fill contains what the label says, and particle size affects both blend behaviour and dissolution. These are non-sterile preparations compounded under USP General Chapter 795 and carry a beyond-use date assigned at preparation rather than a manufacturer expiry.
